Our Supportive Care Program provides an extra layer of support to help you manage your health, stay ahead of your health declining as you age, and feel more supported and confident at home. We work alongside your doctors to bring everything together, so you have the guidance and support you need, when you need it.
Depending on your needs and what services you qualify for, Supportive Care at Blue Ridge Care may include:
- Personalized care planning
- Coordination with your doctors and specialists
- Support navigating the healthcare system
- Palliative Care Services
- Help managing symptoms and medications
- Telehealth visits and remote patient monitoring
- Education and tools to help you manage your condition at home
This program is not hospice. You do not need to be near the end of life or eligible for hospice to receive this care.
Our Supportive Care program may be right for individuals who:

- Have experienced recent hospital visits or health setbacks
- Need extra support managing their care at home
