Safe and Happy At Home With The Care You Need

Aging doesn’t have to mean giving up independence. Our Blue Ridge Independence at Home Program of All-Inclusive Care for the Elderly (PACE) is a comprehensive healthcare program for older adults who need support but want to continue living in their own homes and communities. We coordinate and provide all the care you need, so you don’t have to navigate the healthcare system alone.

At its core, PACE is about helping you live life on your terms, with the support, dignity, and peace of mind you deserve. PACE provides and coordinates the care you need while staying safe and happy at home.

PACE provides and coordinates a full range of medical, personal, and supportive services, including:

  • Primary and nursing care

  • Specialist care
  • Prescription medications, management, and home delivery
  • Medical equipment and supplies
  • Preventative care, including dental, vision, and podiatry
  • Nutritional counseling and meals
  • Physical, occupational, and recreational therapy
  • Behavioral health and social services
  • Home health and personal care assistance
  • Transportation to medical appointments
  • Adult day programs and social activities
  • Hospital and nursing home care when needed
  • Emergency medical services

Your Care Team

PACE is powered by a dedicated interdisciplinary team of healthcare professionals who work together to support your whole health and well-being. From clinicians and therapists to personal care aides and care coordinators, every member of the team plays a role in delivering compassionate, coordinated care. Together, they focus on what matters most — your health, independence, and quality of life.

PACE Center

At the PACE Center, participants conveniently have access to health and personal care services with our interdisciplinary care team on site. Participants are welcome to spend the day with us. We provide nutritious meals and coordinate activities to stay active and socialize.

The PACE Center is open to participants Monday through Friday, from 8:30 AM to 4:30 PM. Transportation to and from the Center is included.
